Receive coaching services through Wisconsin's Children's Long Term Care Waiver (CLTS). Services can be provided under Empowerment and Self-Determination, Family/Unpaid Caregiver Support and Services, Participant and Family Directed Good and Services. Contact Sally to discuss how to add to your service plan. Sally focuses on youth in transition from children to adult services who are using CLTS.
